Parent Notification: Request for Medical Records and Patient Portal Access

 

As you may be aware, COPA now offers an electronic patient portal. The COPA electronic portal is a secure communication pathway between our patients and their primary care provider. For patients under the age of 15, the COPA portal allows the parent to make appointments, update their child’s medical history, request medical records and prescription refills, message their primary care provider, and make payments.

 At the age of 15, by Oregon State law, adolescents become active participants in their own medical decisions. At this time, COPA is not offering portal accounts to any patient 15 and over because as we cannot guarantee the privacy of an adolescent’s health information through an electronic portal.

COPA continues to be able to provide the patient with his or her medical records upon written request. If you have questions about this policy, or your adolescent child’s privacy under the law, please feel free to contact COPA at 541-389-6313.

Prepare For Your Visit

COPA welcomes new patients and families to our practice, whether you have a newborn or child of any age. Just give us a call and set up your appointment at 541-389-6313. We ask that you prepare for your visit by reading additional information listed below. We'll need you to bring your child's medical history with you or transfer medical records before you arrive. We look forward to meeting you!

Appointment times

We ask that our patients arrive 15 minutes before the scheduled appointment. This allows us the best opportunity to see all patients at the correct time. If you arrive late, we may ask that you reschedule for another date.

Our patients are our top priority and the providers make every effort to see you at your appointment time. We give every child our undivided attention and respond accordingly to seriously ill patients. Occasionally, unforeseen emergencies may cause a doctor to be delayed or called away from the office. We hope that you will understand and realize that if your child has an emergency, the physicians are committed to attend to them. That is one of the many benefits of being a COPA patient!

Same Day Appointments for Sick Children

COPA is not a walk-in clinic​; however, we do offer same-day appointments for our regular patients who are sick in the few slots we keep open for that reason. If your child is ill, please call first thing in the morning as these slots can fill up fast and we ask you to be patient as we work to fit your child in.

Insurance (New or Change)

As a courtesy, COPA will submit claims to all insurance carriers if we have received proper verification of insurance coverage at the time of each visit. We are happy to send an invoice to your insurance carrier; however, the patient is ultimately responsible for all charges. Within 60 days of the visit, a payment (by you or your insurance company) must be made on the patient's account to remain in good standing with our office. Please remember that insurance companies often deny claims based on the coverage your policy provides or deductibles that have not been met. If you have any concerns regarding your coverage or insurance payment, please contact your insurance carrier.

No-Show Fees

We encourage all patients to keep their appointment whenever possible. With the overwhelming demand for pediatric services in our community, this allows us to help as many children as possible everyday. For this reason we charge a $20.00 "no show" fee if your appointment is not canceled or rescheduled with at least a 2 hour notification for sick visits and 24 hours for a Well-child check visit. Continued "no shows" may result in your family being discharged from our practice.

Billing

Co-payments and deductibles are due at the time of service. You may receive a balance due statement from COPA after we have received payment from your insurance company or if you have no insurance. It is our expectation that you will pay the balance due within 30 days. We accept cash, local checks, debit cards and credit cards; including VISA and MasterCard.
